Nutrition Support Service
We provide a community based nutrition and dietetic service for adults with a Solihull GP, who are being fed completely or partially by tube at home, including nasogastric, gastrostomy and jejunostomy.
There is also a community-based nutrition and dietetic service for adults who are taking oral diet but have complex feeding problems or are likely to have complex feeding problems leading to a tube placement for feeding. This includes multiple sclerosis, motor neurone disease, Huntingdon's disease, CVA, acquired brain injury, cancer, where the person is either not being seen by another Specialist Dietitian in the region or requires shared care.
We also provide training to Solihull Community staff, GPs and appropriate community health care providers on the recognition and treatment of malnutrition.
These services include:
- Assessment and monitoring of nutritional status.
- Assessment and monitoring of enteral tube feeding.
- Advice on prescribing of nutritional products.
- Delivery of nutrition support training.
- Discharge liaison with tertiary services.
Who can refer to the service?
GPs, Specialist Teams, Community Nurses, Specialist Nurses, Social Services, Mental Health and Tertiary Services can refer to us by completing our referral form.
