What can I expect from the service?
Short term additional support is offered from the Community Diabetes services, depending on your needs or reason for referral. You can benefit from:

How can I get referred into the Community Diabetes services?
A referral form is accepted from a health care professional (e.g. GP, Practice Nurse/District Nurse), which clearly identifies the reason you are being referred.
If you feel you would benefit from the service and would like to know more, ask your GP or Practice Nurse/Specialist Nurse to refer you.
